Medicare's GLP-1 Bridge Offers a 50-Dollar Monthly Pathway to Weight-Loss Drugs Through 2027

A CMS demonstration program launched in July gives eligible Part D enrollees access to Wegovy, Zepbound KwikPen, and Foundayo at a flat 50-dollar monthly copay, with no standard Part D deductible, through December 2027 after a planned longer-term coverage model was postponed indefinitely.

CMS launched the Medicare GLP-1 Bridge on July 1, 2026, as a Section 402 demonstration giving eligible Medicare Part D enrollees a flat-rate pathway to three specific weight-loss drugs: Foundayo, Wegovy in injectable and tablet form, and Zepbound KwikPen. The copayment is 50 dollars per month. The standard Part D deductible does not apply.

The bridge was designed as a short-term transition to the BALANCE Model, a broader program that would have folded obesity-drug coverage into standard Part D plan benefits starting in 2027. In April 2026, CMS delayed the Medicare Part D portion of BALANCE indefinitely, saying it needed to gather more utilization data from the bridge before Part D plan sponsors assumed that risk. The bridge was then extended through December 31, 2027.

Eligible Medicare Part D enrollees can access Wegovy or Zepbound for 50 dollars a month through the GLP-1 Bridge, now extended through December 2027.

Manufacturers agreed to provide the three covered drugs at a negotiated net price of 245 dollars per 30-day supply, according to KFF. Because the program operates outside a beneficiary's regular Part D plan, plan sponsors carry no financial risk for bridge-dispensed prescriptions.

Beneficiaries who already receive a GLP-1 through their current Part D formulary do not qualify for the bridge. The standard Low-Income Subsidy that reduces cost-sharing for low-income Part D enrollees does not apply to the 50-dollar bridge copayment.

With the 2027 open enrollment period opening October 15, enrollees who meet the clinical criteria for the bridge should know that switching or enrolling in a new Part D plan during the annual enrollment period will not affect bridge access, as the program runs independently of plan-level coverage.
