Federal Grant of 11.7 Million Dollars Targets Nursing Home and Long-Term Care in Rural Vermont

The federal government announced 11.7 million dollars in grant funding for Vermont on September 10, 2026, aimed at improving nursing home and long-term care services in rural parts of the state.

The award flows through the Rural Health Transformation Program, a 50 billion dollar federal initiative authorized under the Working Families Tax Cuts Act. The program distributes up to 10 billion dollars per year across all 50 states from fiscal year 2026 through fiscal year 2030. Vermont's award of 11.7 million dollars is directed at three areas: expanding resident access to care, improving physical care facilities, and strengthening the workforce that serves nursing home and long-term care residents.

The nursing home focus distinguishes Vermont's announcement from other state awards in the same program, which have covered a wider range of rural services including ambulances, telehealth technology upgrades, and emergency communications systems.

For Medicare beneficiaries, the investment is directly relevant. Medicare Part A covers skilled nursing facility stays for up to 100 days following a qualifying hospital admission of at least three consecutive days. Staffing levels and facility conditions at nursing homes affect the care available to Medicare enrollees during post-acute recovery.

CMS project officers are assigned to each state to guide implementation and track progress throughout the life of the program.
